​How do I request a Golden Mountaineer Card?
 
If you have a West Virginia Driver's License or State ID card, you will receive a letter and application in the month of your 60th birthday. You can also request a card by contacting the West Virginia Bureau of Senior Services at (888) 968-4462, contacting your local senior center, or request a card online.

 

My card was lost or stolen. How do I get a new one?
 
Call (888) 968-4462 Monday through Friday, 8:00 a.m. to 5:00 p.m., to request a replacement. If after business hours or a State recognized holiday, please leave a message.

I applied for a new or replacement Golden Mountaineer Card but have not received it. What should I do?
 
You should receive your replacement card within 20 days after your request is received. If you have not received your card within this time period, call (888) 968-4462 Monday through Friday, 8:00 a.m. to 5:00 p.m., to confirm your application was received and processed. If after business hours or a State recognized holiday, please leave a message.
